The Role We are seeking a dedicated Health Coach (NBC-HWC) to join our collaborative care team. The Health Coach will play a key role in supporting women navigating perimenopause and menopause by guiding lifestyle changes, providing education, and serving as an accountability partner within a multidisciplinary clinical model.
As a Health Coach at Horizen Medical, you’ll work closely with our Gynecologist, Nurse Practitioners, and clinical staff to deliver a personalized care experience. This is a unique opportunity to be part of a team that values collaboration, compassion, and whole-person health.

Responsibilities Provide one-on-one coaching sessions to help patients create and achieve self-directed health goalsCollaborate with the Gynecologist, Nurse Practitioners, and mental health providers to ensure coordinated, integrated care Use motivational interviewing, behavior change strategies, and evidence-based coaching frameworks to help patients adopt sustainable lifestyle changes Educate patients on nutrition, movement, sleep, and stress management as they relate to perimenopause and menopause Help patients navigate weight management challenges, hot flashes, mood shifts, sleep issues, and other common midlife symptoms with practical, goal-oriented strategies Support adherence to care plans, treatment recommendations, and follow-up visits Maintain accurate documentation of coaching sessions and care plans within the clinic’s EMR Opportunity to contribute to clinic programming, educational resources, and workshops that support women’s midlife health Stay current on women’s health, behavior change, and integrative care best practices through continuing education

Requirements
Qualifications National Board Certified Health & Wellness Coach (NBC-HWC) required
Bachelor’s degree in a health-related field (Nutrition, Exercise Science, Psychology, Public Health, etc.) preferred
Minimum 2 years of experience in health and wellness coaching, with experience in women’s health, endocrinology, or midlife health strongly preferred
Skilled in motivational interviewing and behavior change frameworks
Excellent interpersonal and communication skills with the ability to build trust and rapport
Passion for supporting women through perimenopause and menopause with a holistic, empowering approach
Preferred Qualifications Prior experience in a clinical or integrative health setting
Knowledge of/interest in perimenopause/menopause-related challenges (weight management, sleep, bone health, cardiovascular health)
Familiarity with EMR platforms and collaborative care models
